When AI fails: learning without looking for culprits

To write down what is punished and what is learnt when AI fails.

   What was the last mistake by a program or by AI that someone reported in your company?
2. 2.
   What happened to the person who reported it? Would they report it again?
3. 3.
   Apply the test to that mistake: would another equally competent person, with the same screen and the same rush, have failed in the same way? Which part of the system made it possible?
4. 4.
   Write down the things that are punished in your company when AI fails. Three at most. Everything else is learnt from.
